Registration Documentation and Records +
1. Proof of Age – In order to determine, for instance, the programming needs of your child/children and in compliance with New York Education Law Section 3218, you will need to demonstrate proof of age by providing one of the following:
a. An original or certified transcript of a birth certificate or record of baptism (including an original or certified transcript of a foreign birth certificate or record of baptism) giving the date of birth; or
b. Passport (including foreign passport) giving the date of birth.
If none of the above-mentioned documents are available, the District may consider other documentary or recorded evidence in existence for two or more years, except an affidavit of age.
One or more of these documents may be necessary and consist of the following:
- Official driver’s license;
- State or other government issued identification;
- School photo identification with date of birth;
- Consulate identification card;
- Hospital or health records;
- Military dependent identification card;
- Documents issued by federal, state or local agencies (for instance, local social services agency, federal Office of Refugee Resettlement);
- Court orders or other court-issued documents;
- Native American tribal document; or
- Records from non-profit international aid agencies and voluntary agencies.
Note: The District shall have the responsibility to verify the records if necessary.
2. Proof of Residency – According to New York State Education Law, to attend the schools of the District you must reside at an address within the District’s geographical boundaries. Residency in this context means domicile and requires both physical presence as an inhabitant and the intention to reside within the District. As such, the District requires the submission of documentation and/or information that establishes the physical presence of you, as the parent or guardian, and your child/children.
Please be advised that within three (3) business days of the initial enrollment of your child/children, the District will consider all documentation submitted by you and make a determination as to your residency and the right of your child/children to attend school on a tuition free basis. If the documentation you provided is not sufficient to allow the District to verify your child/children’s residency in the Bay Shore School District, you will be provided an opportunity to submit additional information. If the District then concludes that your child/children is/are not District residents, the District may then exclude your child/children from the Bay Shore public schools. If that occurs, you will be provided with a written notice that explains the specific basis for the District’s conclusion that your child/children are not residents (including a description of the evidence on which that determination was based), the date of your child/children’s exclusion, and information about appealing the district’s determination to the Commissioner of Education.
Section A
1. Copy of a residential lease or proof of ownership of a house or condominium, such as a deed or mortgage statement.
2. A sworn or unsworn statement by a third-party landlord, owner or tenant from whom the parent(s) or person(s) in parental relation leases or with whom they share property within the Bay Shore School District. (Owner/Landlord Statement is included in the registration packet). Only one of the aforementioned statements is necessary.
3. Any other statement by a third-party establishing the parent(s) or person(s) in parental relation’s physical presence in the District.
Specifically, the following other forms of documentation establishing physical presence in the Bay Shore School District, may include the following, as listed below under “Section B”:
Section B
- Pay stub
- Income tax form(s)
- Utility bill or other bills (e.g., power company, cable, National Grid, etc.)
- Membership documents that are based upon residency (e.g. library cards)
- Voter registration document(s)
- Official driver’s license, learner’s permit or non-driver identification
- Documents issued by federal, state or local agencies (for instance, local social services agency, federal Office of Refugee Resettlement)
- Evidence of custody of the child/children, including, but not limited to judicial custody orders or guardianship papers
- Other forms of documentation and/or information establishing parent(s) or person(s) in parental relation physical presence in the School District
Proof of residency is required. To establish your residency within the geographical confines of the Bay Shore School District you should provide at least one item from Section A and two items from Section B. If you cannot provide an item from Section A, you will need to provide three items from Section B.
3. Please complete all Bay Shore School District registration forms.
4. School Records – All records from the school the student is currently enrolled in (i.e. student report cards and/or transcript, NYS test results, and special education IEP or 504 accommodation plan, if applicable). Receipt of such records is not required as a prerequisite for enrollment. However, to facilitate the proper educational placement of your child, please provide us with such records as soon as possible.
5. Health Examination – Proof of a satisfactory health examination conducted by a physician, physician assistant, or nurse practitioner licensed in New York, upon first entering the District, and upon entering prekindergarten, kindergarten, and the second, fourth, and seventh grades. To be acceptable, such examinations must have been conducted no more than 12 months before the first day of school. A Student Health Examination Form is included in the registration packet.
6. Dental Health Certificate (Optional) – The District requests that you submit a dental health certificate for your child/children upon first entering the District, and upon entering prekindergarten, kindergarten, and the second, fourth, and seventh grades. A Dental Health Certificate is included in the registration packet.
7. Immunizations – Proof of immunization in accordance with the age-appropriate schedule recommended by the Advisory Committee for Immunization Practice (ACIP). The school nurse will review and approve immunization records prior to enrollment of new students. No child may be admitted to, or allowed to attend, school for more than 14 days without acceptable evidence of immunization. This 14-day period may be extended, on a case-by-case basis, to 30 days as provided for by law.
8. Parent(s)/Guardian(s) shall provide proper proof of parental relationship – The District may require the parent(s) or person(s) in parental relation to provide the District with an affidavit either: (1) indicating that they are the parent(s) with whom the child/children lawfully resides; or (2) indicating that they are the person(s) in parental relation to the child/children, over whom they have total and permanent custody and control, and describing how they obtained total and permanent custody and control, whether through guardianship or otherwise. Parent and custodial affidavits will generally be required; however, in circumstances where a parent affidavit cannot be obtained (e.g. parent is outside of the country or otherwise incapacitated), a custodial affidavit may suffice, subject to review of the documentation and information provided during the enrollment process. The District may also accept other proof, such as documentation indicating that the child/children resides with a sponsor with whom the child/children has been placed by a federal agency. The District may not require the submission of a judicial custody order or an order of guardianship as a condition of enrollment. 10. The Office of Student Services and Central Registration will not request on any enrollment/registration form(s) or in any meeting or other form of communication any of the following documentation and/or information at the time of and/or as a condition of enrollment:
- Social Security card or number; or
- any information regarding or which would tend to reveal the immigration status of the child, the child’s parent(s) or the person(s) in parental relation, including but not limited to copies of or information concerning visas or other documentation indicating immigration status.
PLEASE BE ADVISED, that in order for your child/children to attend the schools of the Bay Shore School District, you must be a resident of the District. If the District determines at any time that you are not a resident of the District, you will be provided an opportunity to submit additional information concerning your child/children’s right to attend the District’s schools. If the District then concludes that your child/children is/are not District residents, the District may exclude your child/children from the Bay Shore public schools. If that occurs, you will be provided with a written notice that explains the specific basis for the District’s conclusion that your child/children are not residents (including a description of the evidence on which that determination was based), the date of your child/children’s exclusion, and information about appealing the district’s determination to the Commissioner of Education. In such instances, you may be liable to the District for payment of tuition for the period of time that your child/children attended the District’s schools as a nonresident.
PLEASE NOTE, that parents and your child/children may also be required to meet with building personnel before receiving instruction within any school of the Bay Shore School District, but this process will not delay your child/children’s ability to begin receiving instruction on the next school day after you request enrollment.

Nota: El Distrito tendrá la responsabilidad de verificar los registros si es necesario.
2. Comprobante de Residencia – Según la Ley de Educación del Estado de Nueva York, para asistir a las escuelas del Distrito, debe residir en una dirección dentro de los límites geográficos del Distrito. Residencia en este contexto significa domicilio y requiere tanto la presencia física como habitante y la intención de residir dentro del Distrito. Como tal, el Distrito requiere la presentación de documentación y/o información que establezca la presencia física de usted, como padre o guardián, y de su hijo/hijos.

Sección A

Una declaración jurada o no jurada de un tercero, propietario o inquilino de quien el padre(s) o la persona(s) en relación parental alquila o con quien comparte la propiedad dentro del Distrito Escolar de Bay Shore. (La Declaración del Propietario está incluida en el paquete de inscripción). Solo es necesaria una de las declaraciones mencionadas.
3. Cualquier otra declaración de un tercero que establezca la presencia física del padre(s) o la persona(s) en relación parental en el Distrito.

Se requiere prueba de residencia. Para establecer su residencia dentro de los límites geográficos del Distrito Escolar de Bay Shore, debe proporcionar al menos un elemento de la Sección A y dos elementos de la Sección B. Si no puede proporcionar un elemento de la Sección A, deberá proporcionar tres elementos de la Sección B.

4. Para que sean aceptables, dichos exámenes deben ser realizados no más de 12 meses antes del primer día de clases. En el paquete de inscripción se incluye un Formulario de Examen de Salud del estudiante.
6. Certificado de Salud Dental (Opcional) – El Distrito solicita que presente un certificado de salud dental para su hijo/hijos al ingresar por primera vez al Distrito y al ingresar al jardín de infantes y segundo, cuarto y séptimo grado. El paquete de inscripción incluye un Certificado de Salud Dental.
7. Vacunas – Comprobante de vacunación de acuerdo con el cronograma apropiado para la edad recomendado por el Comité Asesor para la Práctica de Inmunización (ACIP). La enfermera de la escuela revisará y aprobará los registros de vacunación antes de la inscripción de nuevos estudiantes. Ningún niño puede ser admitido o asistir a la escuela por más de 14 días sin una evidencia aceptable de vacunación. Este periodo de 14 días puede extenderse, según el caso, a 30 días según lo estipula la ley.
8. Universal Pre-Kindergarten

Eligible students must turn four (4) years old on or before December 1 of the current academic year.
Section 3602-e of Educational Law defines an "eligible child" as a child who resides within a UPK participating school district and who is four-years of age on or before December 1st or otherwise eligible to attend kindergarten the following school year. A child who is age-eligible to attend kindergarten is not eligible for the UPK program.

Kindergarten Registration +
Parents who reside within the boundaries of the Bay Shore School District may register kindergarten-age children at the Office of Student Services and Central Registration. Eligible students must turn five (5) years old on or before December 1 of the current academic year.

Following the registration process, you will be given an appointment for your child’s academic and health screening.

Change of Address/Guardianship+
Any change of address or guardianship requires specific documents be immediately provided to the
Office of Student Services & Central Registration. Please refer to the Registration Process section.
Failure to do so may result in the student’s immediate withdrawal from the Bay Shore School District.

School Attendance

All children between six and sixteen years of age are required by law to attend school. Moreover, regular attendance and punctuality are important to the students’ achievement in school as well as the development of positive qualities important throughout life. To that end, please make note of the following:
ABSENCE: New York State law requires a legal excuse for each and every absence from school or lateness to school. The excuse must be in writing, signed and dated by the parent or legal guardian. In addition, the note must state the specific date(s) and reason for the child’s absence or lateness. The note must be delivered to the designated school official on the day the child returns to school.
New York State Education law permits legal absence from school for the following reasons:
- Illness
- Illness or death in the family
- Impassable roads making travel unsafe
- Religious observance
- Required court appearance.
Please be advised that failure to ensure a child attend school on a regular basis may necessitate a report to appropriate Department of Social Services authorities (Child Protective Services) for investigation and proper action.
In addition to STUDENT CODE of CONDUCT related consequences, failure of a student to attend school on a regular basis may also require a report to the Suffolk County Department of Probation for the institution of a Person in Need of Supervision (PINS) proceeding through the Family Court.

Report a Residency Issue

The Bay Shore School District strives to provide quality educational services while ensuring a safe, supportive and respectful environment for the children of current district residents. Whenever a family moves either within or outside of the district, the move must be reported to the Office of Central Registration as soon as possible. Any move outside of the district requires every student to be withdrawn and registered in their new district of residence.
Unfortunately, and for a variety of reasons, families move out of the district and do not self-report, continuing their children’s enrollment in the Bay Shore School District. These student enrollments are contrary to district policy and unfair to our actual and current resident-students.
If you have any good-faith information regarding currently enrolled students you suspect or know are actually living outside of the Bay Shore School district, please fill out the Student residency Concern form with as much detail possible. All information received is confidential and can be submitted anonymously. The information received will be reviewed and thoroughly investigated by district officials. Your efforts to help us address student residency discrepancies will be well-appreciated.
